Ocps Sports Physical Form – All students who wish to participate in marching band must pass a physical exam and EKG. A physical exam must be completed annually, while an EKG must be performed only once in high school.

All students must register via https://athleticclearance.fhsaahome.org/ (see below). Once the account is created, the form must be uploaded after the student registers.
Forms must be uploaded in time for students to participate before the start of the camp on July 18th.
Bring the physical form and the OCPS cardiology report form to your physical exam and EKG appointment. This form must be completed by a physician. OCPS does not accept other forms of this procedure.

Physical: All athletes, bands, dances and NJROTC members must have a valid physical on a practice permit. The physical exam is valid for one year from the date of signature by the health care provider. The physical examination must be performed and signed by a Florida-licensed health care provider who has no familial relationship to the student-athlete. Only the FHSAA EL2 preliminary form for track and field is accepted. This is not the form used for school registration.
EKG: All athletes, band, dance and NJROTC members must have a routine EKG on their athletic clearance account. See the OCPS Cardiology Report Form below. An EKG should be performed before participating in conditioning or trials. Only one EKG is required per high school experience. An EKG may be completed by a licensed physician, including a primary care physician or pediatrician, or at one of the OCPS School Heart Screen events. If the ECG is abnormal,

NFHS Video Requirements: The FHSAA requires all student-athletes to watch and complete the following NFHS courses annually. After the student registers on the list, the certificate of completion is checked by the coach. Upload each course completion certificate to your practice permit account in the Files section.
